We are sending out a corrected version of this letter. A mistake was noted in the number of doses for the Varicella Vaccine. The correct number of required Varicella Vaccines is two (2).
I would like to welcome your child to Ottawa Township High School starting 2024 - 2025 school year.
It is state law that all students entering 9th grade have a completed 9th grade physical with an immunization record and dental examination on file in the health office. Included in this letter is the State of Illinois Certificate of Child Health Examination form (physical form) and a dental examination form. Please take the physical/dental forms with you to your student’s appointments to be completed by the doctor/dentist.
Physical and Immunizations are due by the first day of school. Minimum immunization requirements include:
Three or more doses of DTP/DTaP
One dose of Tdap
Three or more doses of Polio with the last dose received on or after the 4th birthday.
Two doses of Varicella (chicken pox): first dose on or after the 1st birthday, second dose no less than 4 weeks (28 days)
Two doses of Measles, rubella, and Mumps Vaccines, the first dose on or after the 1st birthday, second dose no less than 4 weeks (28 days)
One dose of Meningococcal Vaccine: first dose on or after the 11th birthday.
Dental exam will be due by May 15, 2025. Dental Exams can be up to 12 months prior to the first day of school on August 8, 2024.
Parents/Guardian are to fill out the health history, sign, and date the physical form. Please include any allergies and/or medication your student has.
You may return the completed physical, immunizations and dental forms at any time to the health office at OTHS, room 205. Or you may fax it to the health office at (815) 431-3350. If you have any questions, you may contact the health office at (815) 431-2461.
